This is part two of the lecture I delivered at the recent Interventional Orthobiologics Meeting in Scottsdales, Arizona. It was a fantastic meeting and delightful to connect with new and old friends.

Part two of biohacking your tendon pain focuses on new diagnostic modalities and novel biologic treatments for tendinopathy including platelet rich plasma (PRP).
Ultrasound has become the preferred imaging tool for patients who present with tendon related pain because it is less expensive than an MRI and can be done immediately at the point of care. Here is an image of moderate tendinopathy and a small spur in a patient with tennis elbow:
Simple initial treatments for elbow tendon related pain include the use of a wrist splint to rest the affected tendons, topical anti-inflammatory medications and forearm strengthening exercises.
Other treatments include shockwave therapy. This treatment uses sound waves to help reduce the symptoms associated with tendinopathy and restore joint function. (See image below)
Dr. Mishra recommends against the use of cortisone for the treatment of tendinopathy based on its lack of long term efficacy and potential for side effects such as dermal atrophy and skin discoloration. (See Below)
Dr. Mishra completed his lecture by discussing the value of platelet rich plasma (PRP) for the treatment of various types of tendinopathy. Tennis elbow has the best published evidence supporting its use including the largest randomized controlled trial of all time testing PRP. (See results below)
Dr. Mishra also presented his new technique for operating on patients with severe elbow tendinopathy that includes the application of PRP at the time of surgery. (See image below)
